Medical Plans – SmartDog provides medical benefits through United Healthcare and offers five plans with coverage options for employee, employee plus spouse, employee plus child(ren) or employee plus family. All options are traditional PPO plans and cover both in-network and out-of-network provisions. One plan is a Health Savings Account (HSA).

Medical Expense Reimbursement Plan (MERP) – SmartDog Services offers deductible assistance for the last portion of their in-network deductible.
Dental Coverage – Comprehensive dental care for all your needs. Guardian Life provides two Dental plan coverage options at the same price. SmartDog has chosen Guardian because of its competitive rates, excellent service reputation, and extensive plan designs. We have worked hard to negotiate group rates that will be affordable for all employees. All coverage is paid through payroll deduction.
Vision Coverage – SmartDog offers VSP Vision coverage for all employees. VSP is the nation’s largest Vision Insurance provider. They pride themselves on supreme customer service, while providing a cost effective benefit to its members.
Basic Life/AD&D – Mutual of Omaha Life Insurance –Protection for you and your family. At no cost to you SmartDog provides Basic Term Life coverage equal to 200% percent of your salary. Basic Term Life coverage includes Accidental Death and Dismemberment coverage equal to the life benefit payment.
Long Term Disability – At no cost to you, SmartDog also provides Long Term Disability coverage of up to 60% percent of your annual salary. Disability payments provide a regular check to help pay monthly bills when you are unable to work. Long-term disability benefits start within months of the illness or injury, but may continue until retirement.
Voluntary Life and STD Coverages – From Mutual of Omaha, elections can be made for Voluntary Term Life coverage for you and your family. Also, Short Term Disability coverage is also available for up to 60% of your weekly salary. We have negotiated very low premiums to allow you to provide the peace of mind that Life and STD coverage(s) can provide.
Voluntary Supplemental Coverages– You may elect additional voluntary supplemental coverages for Accident, Critical Illness and/or Cancer Assurant. They are a national provider of premium voluntary, portal benefits. Convenient payroll deductions offer access to these supplemental products.
Flexible Spending Account (FSA) – Through Ameriflex, SmartDog offers the option of a Medical FSA to help you to save BIG on healthcare expenses that are not covered by your medical, dental and vision insurance plans. Significant tax savings can be gained when using a Medical FSA for uncovered expenses, such as deductibles and co-payment costs. SmartDog also offers a Dependent Care FSA, which provides a tax savings solution for the high cost of this necessity.
401K Retirement– SmartDog provides a 401K Retirement plan to all eligible employees, beginning the first of the month after date of hire. This benefit is serviced through Fidelity NetBenefits Center. All employees are encouraged to thoroughly consider the benefits of saving for their retirement needs.
